The form presented here is the Blood Bank Order form. It is used by an attending physician to order (requisition) Blood products to be made available for use in a surgical procedure, on the date stated on the form. The analysis of this form gives a glimpse of just how well controlled the valuable resource of Blood actually is. This form, or one similar to it, is fully completed before or during every surgery anticipating the need for Blood. No Blood moves without a form similar to this one.
